Fatigue and 3am Waking
By Meghan Swidler, Functional Health + Detox Practitioner · Updated October 27, 2026
Meghan has guided hundreds of clients through her proprietary five-phase method since 2021: drainage, then the gut, the liver, parasites, and mold and heavy metals, in the order the body heals.
Why am I always tired even after 8 hours of sleep?
When a full night of sleep doesn't restore your energy, Meghan looks upstream at the liver, gut, nervous system, blood sugar and drainage pathways. The drivers she sees most:
- A liver doing heavy overnight work without enough support.
- A nervous system stuck in fight-or-flight, which lowers stomach acid and bile so nutrients don't absorb.
- Mitochondria short on the minerals and B vitamins they need to make energy.
- T4 that isn't converting to active T3, often because of gut problems.
- Blood sugar that spikes and crashes, and under-eating, which the body reads as scarcity.
- Mobilizing toxins faster than you clear them through fasting, juicing, intense workouts or detox supplements.
- Toxic burden from heavy metals or mold, and sleep broken up by airway problems.
What to do:
- Eat enough at regular meals. Get your bowels moving daily and your drainage open before any fasting, juicing or detox supplements.
- Protect your sleep, since much of the body's repair happens overnight: be asleep by 9 to 9:30pm in a room at 68 degrees F or cooler, finish eating 2 to 3 hours before bed, put screens away for the last hour and keep the room dark.
- If you menstruate, ease up on intensity in the luteal and menstrual phases.
- Ask for labs a standard panel skips: a full thyroid panel including free T3, ferritin and a full iron panel, B12, folate, vitamin D, fasting insulin and A1c. She treats ferritin below 50 as a problem, even when it's labeled normal.
If you wake unrefreshed, wake between 1 and 3am regularly, or someone notices unusual breathing while you sleep, see a sleep specialist in person. Coffee covers the signal and leaves the cause in place. Her free Energy Audit shows where your energy is going and what to address first.
Why do I wake up at 3am every night?
Meghan checks 3 things first: the liver, cortisol and blood sugar. In Traditional Chinese Medicine, 1 to 3am is the liver's time, and Meghan often sees a liver overloaded by alcohol, toxins or sluggish elimination show up as waking in that window. Chronic stress can flip your cortisol rhythm so it runs high at night, which pulls you out of deep sleep with a racing mind. If dinner was mostly sugar or refined carbs, or you skipped it, blood sugar can crash overnight and release adrenaline that wakes you up wired. In her practice, parasites can play a role too, especially when night waking comes with teeth grinding, bloating and sugar cravings. What to do:
- Eat a real dinner, and finish eating 2 to 3 hours before bed. Stop liquids 2 hours before bed.
- Be asleep by 9 to 9:30pm so deep sleep lines up with the liver's window. Keep the room cool and dark, use only red light in the evening and put screens away for the last hour.
- Cut back on alcohol, and get your bowels moving daily so the liver has somewhere to send what it processes.
- Add liver-supportive foods like beets, dandelion greens, arugula, lemon and cruciferous vegetables.
If the waking comes with snoring, unrefreshed mornings or breathing changes someone has noticed, see a sleep specialist in person. Liver support belongs to Phase 3 (Release) and parasite work to Phase 4, both after drainage, so her free Detox Phase Quiz is a good place to find your starting point.
Why am I still tired on thyroid medication?
When thyroid medication doesn't fix the fatigue, Meghan looks at the gut. Gut health affects the conversion of inactive T4 into active T3, so TSH can look normal while the body is short on active thyroid hormone. Selenium, iodine and zinc, the nutrients the thyroid relies on, are absorbed through the gut lining, so an inflamed gut limits them however much you take. Gut imbalances also drive inflammation that strains the thyroid. Signs she connects to this pattern:
- A normal TSH with ongoing exhaustion
- Hair loss on the "right" dose
- Irregular cycles
- Weight that won't move
- Brain fog and cold hands
Her steps:
- Ask for a full thyroid panel: TSH, free T4, free T3, reverse T3 and antibodies
- Open drainage and get the bowels moving daily
- Rebuild the gut in Phase 2 before adding thyroid supplements
- Keep working with your prescriber, and never change medication on your own
Her free Energy Audit is a good starting point.
Should I quit caffeine?
Meghan sees caffeine as a drug most people overuse. It borrows energy from later in the day, raises cortisol and disrupts deep sleep, with a half-life of about 6 hours. She switched from 2 cups of coffee a day to matcha in 2022 and later let go of caffeine completely. The switch alone brought headaches, brain fog and fatigue for a stretch. Since quitting, she sleeps more deeply, feels steadier and no longer needs it for energy. Her view is that you can't see what caffeine is doing until you take a break from it. If you want to try:
- Have your warm lemon water with sea salt before any caffeine
- Step down gradually, coffee to matcha to caffeine-free
- Try alternatives like cacao, dandelion root tea or mineral drinks
- Expect a rough stretch while your body adjusts
